Think about this: You are nearing the end of a stay in the hospital after an illness or receiving some sort of procedure. As the staff prepares your discharge, you are given medication, follow-up appointment instructions, referrals to physical or occupational therapy, and guidance on how to ensure the success of your recovery. Then, someone who loves you picks you up from the hospital and takes you home and takes care of you for the next few weeks and makes sure you are following your discharge plan. There is not a physician or nurse at your home to make sure this happens, just people who care about you. Our guests don’t have that. That is what our medical respite program provides.

The Gathering Inn (TGI) plans to open a medical respite program at 1660 3rd Street in Lincoln, California, which will contribute to reducing homelessness and the strain on our local healthcare and social service systems. The Gathering Inn was required by the seller's terms to keep the real estate activities confidential until close of escrow, which happened on August 13, 2024.
Medical respite services help ill or injured unhoused patients recover after hospital discharge under medical supervision. In this controlled setting, they also receive support services to transition to stable housing.
Patients in the Medical Respite Program are typically senior citizens. They must be referred by a hospital, and no walk-in, sex offenders or self-referrals are accepted. This is not a shelter or a drop-in day program. Until earlier this year, the building was an assisted living facility. The primarily elderly patients served by the Medical Respite Program will be inside the building or its internal courtyard, and no visitors will be allowed.
We understand and respect the confusion and concern that surrounds a private deal. Now that we have the freedom to do so, we will make every effort to listen to, and engage with, nearby residents, businesses, and City of Lincoln officials.

The Gathering Inn (TGI) is recognized by the National Institute for Medical Respite Care as one of only 21 distinguished programs nationwide. This honor highlights our commitment to excellence and leadership in medical respite care and our role in shaping best practices in the field.
TGI currently manages the only medical respite programs in Placer County, which is not large enough to meet the demand for services. In fact, more than 200 people needing medical respite care were turned away last year alone. That is why TGI sought a location that would make it possible to expand our medical respite program, which is a vital part of the solution to homelessness and to controlling health care costs.
Medical respite services contribute to a healthier, more stable community by reducing emergency room visits and hospital readmission rates and lowering the overall cost of healthcare. These cost-effective services help people successfully recover enough from illness or injury to take advantage of comprehensive support and transition to stable housing solutions.

TGI’s medical respite program has proven to be successful in reducing homelessness.. In 2023, 83% of our guests in the medical respite program went to positive housing destinations after they discharged from our services. Only 5% of guests in the medical respite program returned to the hospital for the same issue.
This program aims to reduce homelessness, not increase it. Last year, out of the 82 guests served in our Medical Respite Program, 10 were from Lincoln. Each of those guests, who were experiencing homelessness when they entered, successfully transitioned to positive housing outcomes upon leaving our program. This shows our commitment to provide care and support lasting, positive changes in the lives of those we serve. We believe this aligns with what Lincoln residents want—to see fewer people experiencing homelessness in their community. By helping individuals recover and transition into stable housing, our program contributes directly to this goal.
Historical data from our medical respite program operations show a minimal impact on law enforcement resources. In fact, only five law enforcement calls about or from our medical respite program were recorded in the past five years.
TGI’s medical respite facility will be located where an assisted living facility has been operational for decades. We expect there to be no change in the frequency of emergency calls to the facility in the neighborhood due to the introduction of the medical respite program.
We understand that the community has concerns about the safety of local residents, including the seniors living across the street from our new medical respite facility and the children attending schools nearby. We want to reassure you that our program is designed with the utmost consideration for public safety and will not operate as a drop-in center or a shelter.

Our program has a proven track record of safety; we have received no complaints from neighbors at our current location and minimal calls to law enforcement over the past five years. We are confident that this will continue in Lincoln, where the new facility will operate under similar guidelines and practices.
We also believe in fostering positive relationships with the local community. To that end, we will offer opportunities for intergenerational volunteering, where students, parents, and seniors can engage with the program in meaningful ways. These activities can help build understanding, trust, and a stronger sense of community.
Admission to this program requires a hospital referral; no walk-in admissions or sex offenders are allowed.
Once referred, guests are safely and directly transported to the facility through a coordinated effort between TGI and medical transport services.
TGI’s medical respite program serves people experiencing homelessness who require a supportive environment for recuperation after hospital care. These individuals – often frequent visitors to emergency departments – benefit from our medical respite program, with an average guest age of 62 years and an average stay of 35 days.
These numbers highlight our commitment to supporting the older homeless population. For example, in 2023, we served 208 people aged 55 and above, including 66 people over the age of 65. TGI’s other services are provided to people age 18 and older.
Expanding TGI’s medical respite program responds to growing demand for medical respite services and a limited number of medical recovery spaces. We identified an appropriate location in Lincoln that will enable us to provide comprehensive care to the people who need it.
This new, larger medical respite facility will help reduce emergency room visits and re-hospitalization by homeless residents and will help reduce the overall number of unhoused people in the region.
TGI has successfully operated medical respite programs in Placer County since 2009, but with 200 individuals turned away each year due to limited capacity, there is a clear need for expansion. TGI acquired a building in Lincoln that previously served as an assisted living facility, which offers the perfect environment to increase medical respite services. This facility offers space to serve more people who need medical respite care and enhances our ability to provide holistic care.
Capitalizing on economies of scale, the expansion is driven by a blend of available funding, an ideal location, and the pressing need. It ensures a cost-effective and efficient solution compared to scattered-site options. The medical respite program is planned to integrate seamlessly into the neighborhood, preserving the community's character and safety.
The medical respite program is primarily funded by the State of California. In 2023, TGI was awarded funding from the California Community Care Expansion Program. Additional funding may come from philanthropic donations and Medi-Cal.

The California Community Care Expansion Program (CCE) funds the acquisition, construction, and/or rehabilitation of adult and senior care facilities that serve applicants and recipients of Supplemental Security Income/State Supplementary Payment (SSI/SSP) or Cash Assistance Program for Immigrants (CAPI), who are at risk of, or experiencing, homelessness.
CCE’s $6.4 million grant to TGI made it possible to relocate and establish infrastructure for the medical respite program. This grant signifies TGI’s alignment with statewide efforts to enhance healthcare access and housing stability for California's vulnerable populations.
Notably, the CCE funding brings with it specific waivers from local jurisdiction regulations that significantly facilitate the expansion process, including:
TGI is required to use Community Care Expansion funding only to enhance and expand care facilities for adults and seniors at risk of homelessness. Repurposing the medical respite program for unrelated uses would not align with these guidelines.
Under the Community Care Expansion (CCE) funding, as outlined by the California Department of Social Services, projects are intended to preserve, expand, or enhance facilities that serve adults and seniors, including those who are homeless or at risk of homelessness.

Historically, TGI has actively served Lincoln residents through our programs in Roseville and Auburn. For example, 83 of the individuals we served in the last 5 years were from Lincoln, underscoring our service to the Lincoln community. This new program location will continue our mission of providing care and support to those in need, with a significant history of serving Lincoln residents alongside the broader Placer County community.

Our search led us to the Lincoln property that is ideally suited for a medical respite program to meet Placer County’s needs. The legislation and funding for this program prevents local jurisdictions from obstructing such initiatives and exempts such projects from City or County review. The State of California removed barriers to providing services to homeless individuals, ensuring that our actions are legally compliant and aligned with our commitment to serving people in need. The medical respite program reinforces our critical role in supporting community health and well-being.
During their stay with us, guests work closely with a case manager who helps them remove barriers to homelessness and equips them to achieve sustainable housing. Last year, 83% of the 90 guests we served went to positive destinations, which include permanent supportive housing, transitional housing, emergency shelter, rehabilitation facilities, or skilled nursing. Of the remaining 17%, only 5% returned to the hospital for the same issue.
For those few who do end up returning to an unsheltered status, all efforts are made to develop a safety plan, including a return to their area of origin. For example, if they were previously living in Auburn, they are transported back to Auburn. We also work closely with all outreach entities in the community and county, collaborating with them on the very few discharges that are not to improved destinations. It is our intent to preserve the neighborhood's character and take all measures to guarantee safety for the community.
Potentially hiring local team members and vendors can support the community by creating job opportunities and stimulating economic activity. This approach can contribute to a stronger local economy while enhancing our ability to provide responsive and effective care through the Medical Respite Program.
